BUTLER COUNTY —Law enforcement authorities and the Occupational Health and Safety Administration are investigating a fatal work place incident at petroleum refiner Holly Frontier 1401 Douglas Road in El Dorado.
On February 18, a 31-year-old contract worker from Alvin, Texas was working in a confined space when other crew members lost contact with him, according to a statement from the Butler County Sheriff's office.
Emergency crews on site recovered the man and he was transported to a local hospital where he pronounced dead. The sheriff's office released no additional details.
